Crohn¢¥s Disease


Abstract
We had experienced 11 cases of Crohn¢¥s disease of the past 12 years from 1972 to 1984 at surgical department of St. Benedict hospital.
We reviewed all medical records, and analyzed clinically with results as follows. 1) Male to female ratio was 1.7: 1.
2) Average age of onset was 40 years, which was more older than the caucasian.
3) The most common symptoms were abdominal cramp, epigastric discomfort, bowel habbit change, anorexia, weight loss, febrile sense, rectal bleeding in orders.
4) The most common physical signs were abdominal tenderness, distention, rebound tende-rness, rigidity, palpable mass on RUQ in orders.
5) Intestinal obstruction was the commonest preoperative diagnosis among this study.
6) All most of the lesion located on small intestine, especially ileum, and one case occurred colon alone and one case involved both small and large bowel.
7) Macroscopic findings were edema and thickening of bowel wall, segmental stenosis and proximal dilatation, thickenning of mesentry, serosal exudate; fat creeping and skip areas in orders.
8) Microscopic findings were submucosal edema, ulcer of mucosa, granuloma, lymphoid hyperplasia and fibrotic change with infiltration of inflammatory cells.
9) All of these patients received operation as follows, segmental resection on nine patients, one has right hemi colectomy with ileo-transverse colostomy. The other one has limited multiple resection.
10) We experienced three cases of complicated Crohn¢¥s disease one of massive hemorrhage, and the other of free perforation, and chronic perforation.
11) As postoperative complication, one patient had intestinal obstruction and the other
patient was expired of panperitonitis due to suture line dehiscence.
